Ryan White Title II HIV Case Management
Ryan White Title II Case Management Program links individuals with HIV or HCV to health care, psychosocial, and other services. Under contract with MDHSS, Rain is one of five case management contractors that provide Title II case management in the Central Region. Case management services are available to any Missouri resident who is HIV or HCV positive. Services are also available for incarcerated or recently released PLWH.
The goals of the case management program are to improve health outcomes by providing support to individuals with chronic illness and help them to become independent.
