HIV Case Management Services
Rain has been awarded the Case Management contract from the Missouri Department of Health and Senior Services to provide Ryan White Title II HIV/AIDS case management services since 1998. This program provides supportive services, referrals and medications to more than 200 people living with HIV/AIDS each year.
Transitional Case Management for Incarcerated PLWH/A:
This program serves almost 300 PLWH/A in prisons across the state, providing pre and post-release prevention counseling, linkage to health care, medication continuation assistance and resource referrals.
Prevention & Education Programs:
This program serves over 7,000 people each year, providing tailored interventions that specifically target prioritized risk groups, including people living with HIV/AIDS. Contact Dustin Hampton dhampton@missourirain.org to schedule a presentation for your group or for more information.
The HIV Nutrition Project and Emergency Food Program:
This program provides nutritional resources specifically for people living with HIV/AIDS, serving 228 unduplicated persons each year.
